Developed through a thorough assessment, data analysis, and research, this dynamic document is based on recommendations from healthcare professionals and peer-reviewed literature. The plan provides a structured approach to addressing all aspects of care, including medications, therapies, equipment, home maintenance, vocational needs, transportation, and childcare. The plan projects the needs of the individual, as well as the associated costs.
Each Life Care Plan includes:
